For some kinds of therapy (like ICT or CSII) you
have to know how many carbs do you take, because your bolus insulin-dose depends on it!
For other therapies like CT you
should know how many carbs do you take; especially, if your beta-cells doesn't produce insulin anymore.
You can't take everyday the same dose of insulin if you you eat sometimes more, sometimes less carbs. As more carbs do you eat, as more insulin is necessary to cover it. (and keep your BG at normal levels)
